Palm View Rehabilitation seeking Director of Social Services. Duties include: Ensure the medically related emotional and social needs of residents are met. Develop comprehensive social history and psychosocial assessments. Assist residents and thier familities to cope with transition to the facility. Develop individualized care plans and facilitate the social and psychological well-being of residents and their families. MSW and unencumbered AZ state licensure, required. Previous experience, preferred.
