Maple Grove  is a 60 bed Long Term care facility in Lebanon. We provide individualized care for our residents with extensive rehabilitation, specialized treatment and 24 x 7  nursing supervision . Maple Grove is the Sole recipient of the 2008 Virginia Health Quality Center Gold Star award.
